I acknowledge that my Personal Data will be utilized exclusively for the provision of healthcare services, treatment coordination, maintenance of medical records and processing of payments for healthcare services. I am aware that my Personal Data will be maintained in strict confidence and will only be disclosed to authorized personnel involved in my care, unless mandated by law.
